Summary:

• Shared care requires accurate disease registers.
• IT facilitates: (a) Recall, (b) Regular review
• Monitoring of quality is essential: (a) Practice level, (b) Pay-for-Performance (P4P) provides some insight, (c) Need to start to think community resources

Current disease registers are good but scope to improve quality.

• Good data quality (around 85% of data is complete and accurate)
• Scope to improve quality further: (a) systematic checks of medical records, (b) when people with diabetes attend for their annual review
• Why it matters: (a) miscoded people may miss out on recall, (b) misclassified can get the wrong treatment & education and (c) misdiagnosed my suffer stigma, driving restrictions, more expensive insurance etc.
